What to Expect
What does the Assessment & Referral Program process look like?
You make the call
- When you are ready to get started, you can call us at (720) 696-7760 or complete this simple referral form and we will reach out to you to discuss your needs.
- Once it is determined an assessment would be helpful for you, our team will work with you to schedule an assessment appointment.
- Prior to an assessment, you will complete a short information packet. If insurance copays or self-pay fees are applicable, these will be due and collected at the time of service.
Your Assessment Appointment
- Your licensed assessment clinician will explain the assessment process and answer any questions you have. Be prepared to discuss the following items:
- Why are you seeking an assessment – what is your goal of getting an assessment?
- Your complete history, including family/genetic history as it applies.
- Symptoms: how long it has been happening, how it is impacting your daily life and relationships?
- Any trauma experienced which can include any experience that has negatively impacted your mental health?
- We will complete a risk assessment as well as a substance use assessment.
- Education and employment status and history.
- Cultural values and experiences
- You may receive additional screening forms to complete prior to your appointment.
After you complete the assessment
- If care is recommended you will be offered an appointment with our Care Connection Specialist. They will review your recommended treatment and discuss available options/resources. The bulk of this conversation will be identifying your preferences in a provider, including location, availability, etc.
- Once you have chosen your top priority providers, your Care Connection Specialist will work with those agencies to assist with your treatment Assessment & Referral Program. This may include sending your assessment to that agency for their reference and referral. Please note: Some agencies may require additional paperwork for your initial appointment after assessment.
- If care is not recommended or you requested a specialized assessment for your treating physician or care provider: With legal permissions, we will send your assessment directly to your treating provider, or we can connect you with our Medical Records department to request a copy of your assessment for distribution at your discretion.
